What you will learn

Codingforhealth’s CRC preparation is designed to train students in the essential foundations of risk adjustment coding, clinical documentation, and ICD-10-CM diagnosis coding. Through the program, students build a solid understanding of how medical conditions, provider documentation, and risk models affect clinical, financial, and reimbursement accuracy within the healthcare system.

During the training, students work with risk adjustment concepts, HCCs, medical documentation, chronic conditions, official ICD-10-CM guidelines, and hands-on scenarios similar to those used in the industry. The goal is for students to identify reportable diagnoses, recognize documentation gaps, and correctly apply the diagnosis coding principles used in risk adjustment.

Risk adjustment coding training objectives

• •Define the main risk adjustment models, including HCC, CDPS, HHS-ACA, and hybrid models.
• •Understand how diagnosis coding affects payments, clinical accuracy, and reimbursement models.
• •Identify chronic conditions and diagnoses that can affect a patient’s risk score.
• •Apply the official ICD-10-CM coding guidelines in risk adjustment scenarios.
• •Recognize key elements of the medical record needed to validate a diagnosis.
• •Identify clinical documentation gaps and understand how they affect diagnosis capture.
• •Review and assign the appropriate ICD-10-CM codes for provider-documented diagnoses.
• •Understand the relationship between clinical documentation, diagnosis coding, and risk adjustment reimbursement.
• •Analyze conditions common in risk adjustment, including diabetes, cardiovascular diseases, CKD, COPD, obesity, cancer, behavioral health, and other chronic conditions.
